Cost is the most common reason people in working-class Las Vegas neighborhoods go without mental health care. Not stigma, not disinterest — money, and the reasonable assumption that therapy is something other people can afford.

So this starts with the practical part rather than ending with it.

The real cost, and how people afford it

Nevada Medicaid covers behavioral health, usually with no copay and no deductible. Nevada expanded Medicaid, which means eligibility is based mainly on income and does not require having children or a disability. A great many working people in central Las Vegas qualify and assume they do not — particularly part-time, hourly, and tipped workers. You can apply any time through Access Nevada; there is no enrollment window.

Nevada Health Link plans, including Silver Summit and Ambetter, include behavioral health, and subsidies often bring premiums far below what people expect.

Reduced-fee sessions. Our associate clinicians see clients at $125 rather than $150, and we have a graduate-level clinician at $25 to $60 per session.

Your employer's EAP. If you work for one of the large resort, hospital, school district, or county employers, you likely have one — usually several free sessions with fast access, and most employees never use it.

If none of those apply, say so when you call. We will tell you the real number before you commit to anything.

Depression in someone who still shows up

Most people picture someone who cannot get out of bed. Far more common is someone who gets up, works the shift, feeds everyone, and feels nothing.

  • Flatness rather than sadness — nothing is enjoyable, including things that used to be
  • Exhaustion that sleep does not touch
  • Irritability, a short temper with the people you love
  • Difficulty concentrating or making small decisions
  • Appetite up or down, weight changing without trying
  • A voice telling you constantly that you are failing
  • Thoughts that people would be better off without you

This is not a failure of character

Worth saying plainly, because the belief that it is keeps people from getting help.

Depression involves real changes in how the brain processes reward, motivation, and threat. It is influenced by genetics, chronic stress, sleep, physical health, and circumstances — with financial pressure and job insecurity among the strongest environmental contributors there are.

None of that is fixed by trying harder, and telling a depressed person to think positively is roughly as useful as telling someone with a broken ankle to walk it off.

What the treatment consists of

Behavioral activation is usually where we start and often the most effective piece. Depression removes motivation, so waiting to feel like doing something means never doing it. The treatment inverts the order: schedule small, specific, achievable activity first and let mood follow behavior. It has strong evidence and does not require being articulate about feelings.

Cognitive work on the patterns depression generates — the certainty that things will not improve, the reading of every setback as evidence about yourself.

Sleep, because disrupted sleep both causes and worsens depression and is often the fastest lever available. For anyone on a casino or hospital shift schedule, this is usually the first target.

Addressing the actual circumstances. If the depression is driven by an unmanageable financial situation, an unsafe relationship, or a job that is grinding you down, therapy that ignores that is not honest.

Coordination on medication where indicated, with a physician or psychiatric provider.

Spanish-language care

Central Las Vegas has a large Spanish-speaking population, and there is a genuine difference between describing what is wrong in your second language and describing it in your first.

On confidentiality, which matters in mixed-status households: nothing you say leaves the session without your written permission, we do not report immigration status to anyone, and you are not required to provide a Social Security number to receive therapy.

Both of our offices serve central Las Vegas — E Russell Road in the east valley and N Durango Drive in the northwest — and we provide secure video anywhere in Nevada, which costs nothing in gas or parking.

Small beginnings are worth something

If therapy is not possible this month, three things have real evidence and cost nothing: get outside in daylight within an hour of waking, move your body for twenty minutes most days, and tell one person the truth about how you have been.

None is a substitute for treatment. All of them help, and they are available today.
